Olio is designed to transform how skilled nursing organizations manage patient transitions; introducing advanced digital referral coordination, centralized care management, and a robust process to reduce hospital readmissions—empowering providers to deliver safer, more effective patient journeys.

In April 2025, CMS broadened its authority to penalize providers for improper transfers and discharges and issues within the transfer and discharge planning process. The implementation of F627 and F628 will significantly change how facilities manage these situations, and facilities that are unprepared may face serious consequences.

“Olio elevates our ability to deliver safe, coordinated transitions for patients leaving our skilled nursing facilities,” stated Janean Kinzie, VP of Social Wellness & Enrichment at American Senior Communities. “The modernized referral process streamlines coordination with providers, while robust documentation and proactive interventions enable our teams to focus on what matters most—keeping patients safe and preventing unnecessary hospital readmissions.”

Olio enables Skilled Nursing Facilities to:

  • Seamlessly coordinate patient transitions across the care continuum
  • Minimize hospital readmissions with a safe return-to-skilled care process
  • Foster transparency and accountability with their home health network
  • Decrease operational costs for post-discharge coordination
  • Establish care coordination as an organizational priority to drive referrals

"As a former SNF VP of Business Development and Managed Care, I know firsthand that robust, actionable data isn’t just a ‘nice to have’—it’s a must-have for SNFs to thrive under increasing value-based payment models and regulatory scrutiny,” stated Troy Reiff, VP Strategic Initiatives at Olio. “Olio now puts SNFs in the driver’s seat for their future success by empowering leaders to proactively manage compliance and performance, with the right data in their hands exactly when and where it matters most."

“This expansion reflects our unwavering commitment to empowering skilled nursing operators with the tools they need to deliver safer, more seamless care transitions,” stated Ben Forrest, CEO at Olio. “As CMS regulations evolve, Olio is proud to help our partners stay ahead- minimizing readmissions, enhancing compliance, and ultimately improving patient outcomes. We look forward to driving meaningful change together in this critical sector of healthcare.”

Olio is now available for Skilled Nursing, launching today with 69  American Senior Communities facilities throughout Indiana discharging to Home Health, Assisted Living, and Hospice.
